Child Registration Form (0-12 years old)

Welcome. We are so glad you would like to join us for the Annual Retreat. Please fill out one registration form for each child that will be coming along with you in case your child gets lost or there is an emergency. Once you have submitted one form and can view the confirmation screen, you can reuse your link to fill out another form.

Please note:
We have 40 spots available in our Children’s Program.
Children’s programs are filled on a first come, first served basis.
There is no additional cost for the Children’s Program.
Children 3+ must be toilet trained to attend programs.
If your child is under 3 we strongly encourage to organize childcare within the group you are travelling with so everyone can enjoy some downtime. Please complete one form per child. We use these registrations to plan food, programs and to identify any missing persons in the case of an emergency.

Allergies / Dietary Notes
We offer a lacto-vegetarian yogic diet (no meat, fish, eggs, garlic or most onions). While can accommodate many dietary requirements, we do use nuts, soy, gluten and dairy in our kitchen. Please list all dietary allergies or conditions that require medical attention and their severity (ingesting vs. being in contact).
It is your responsibility to ensure that any medications, medical appliances or medical alerts are provide by you for your child. For example, inhalers, epi-pens, medic alert bracelets, etc.
